To complete an application, you must do both of the following steps:

1. Download the application form (below) and mail the completed form to

   Drop the application off in person at 1087 Queen Street West (entrance is alley just south of Queen @ Dovercourt) from January 28 - February 1, Monday to Friday, 10:00AM-5:00PM
